Formbricks — self-hostable surveys for in-app, web, link, and email feedback

About these scores

OSS scale score is an unbounded metric that log-compresses and weights Stars, Watchers, Forks, and Contributors. Discovery score is the current OSS scale score minus the score at discovery. Update pace is commits in the last 30 days, growth momentum is the OSS scale score difference within the recent observation window, and OSS health is a 0–100 rating based on available recency, Community Health, and release data.

Overview

Formbricks is a survey and experience-management platform for creating surveys without code and delivering them in apps, websites, shared links, and email. It offers a hosted cloud and Docker-based self-hosting, with audience targeting and response analysis. The latest release checked is 5.4.3.

Features and best fit

Based on official documentation; not hands-on tested · Content checked:

Create surveys and target them to the right users

Multiple question types and templates support survey authoring, while delivery can happen in-app, on websites, through links, or by email. Targeting can place surveys in selected user journeys without requiring a separate application change for every campaign.

Analyze responses and connect other tools

Formbricks includes response-analysis capabilities and documents integrations with services such as Slack, Notion, Zapier, and n8n, bringing collection, sharing, and analysis into one workflow.

For product and experience teams collecting feedback throughout a user journey

It fits teams that need more than a standalone contact form and want surveys tied to product usage or customer touchpoints. Self-hosting also makes it relevant where response-data location needs tighter control.

Review response-data handling and the AGPL/Enterprise licensing boundary

The core is AGPLv3, while apps/web/modules/ee uses a separate Enterprise license and requires a valid Enterprise license for production use. Surveys can contain personal or free-text data, so storage, access, and retention need deliberate controls. Release 5.4.3 also includes a critical dependency security update, making patching policy important for self-hosted deployments.

Supplemental curator note

Formbricks goes beyond standalone forms by collecting feedback throughout a user journey. Self-hosted evaluations should include response-data protection and the boundary between AGPL core features and Enterprise modules.
